6 Buttons on a Sega I/O

Can anyone show me what I will need to output 6 buttons on a Sega I/O board and where it will hook up on the board as well?

do a quick search of the forums...I am 99% sure there is a post already that explains it. I recall looking it up when I got mvc2 for my wife.
